The Community C.A.R.E. system is a free online resource that allows Moore County families to ensure that they are receiving all the services they need to be happy, healthy, and successful. Completing this Profile will identify your family's specific needs and help you meet those needs by connecting you to agencies and resources within our community.

Getting connected to local agencies and resources is easier than it has ever been!  Follow these simple steps to create a Community C.A.R.E. Profile and develop your personalized Success Plan;

 

1. PROFILE

A Profile is a series of questions which help identify services that you and your family may need.

 

2. ANALYSIS

Once the Profile has been completed, the Community C.A.R.E. system will analyze your Profile and provide you with a list of services that you might find useful, along with an explanation of what each entails. From that list, you may choose the services that you find most useful. 

 

3. SELECT PROGRAMS

Based on your selection of services, the Community C.A.R.E. system will generate a sorted list of programs in Moore County that can help you receive those services.  You are encouraged to review the list and select the      programs you think will be able to help you and your family receive the services you need. 

As you make your selections, you may also choose to share a copy of your Profile with each program you   select.  Doing so would allow those programs to follow up with you and address any questions you might have. 

 

4. SUCCESS PLAN

After you complete your selections, a Success Plan will be developed exclusively for you.  Your Success Plan will list information about each program you selected and can be used to help you meet your family’s needs.

 

Our goal is to make receiving services for you and your family as simple and hassle-free as possible, all while ensuring your privacy and freedom of choice.

The Mobile Messenger (M2)

 

The M2 is a mobile information hub for Moore County families.  It is equipped with computers, internet, and printers so you can create a Community C.A.R.E. Profile and Success Plan wherever you are!
